a cover letter that clearly states the organization's project or service
and the amount requested,
2.)
a brief history of the organization's origins and its current programs,
3.)
a description of the project and personnel, the need and specific objectives
to be addressed, the implementation plan, and the relevance of the program
to the community,
4.)
a list of the Board of Directors,
5.)
a projected calendar of the proposed project,
6.)
a budget for the requested funds -- the amount requested and its purpose
must be outlined in the budget,
7.)
a copy of the organization's IRS letter of Tax Exempt Status 501(c)(3),
8.)
a copy of the organization's current financial statement (audited, if
available),
9.)
a total of six collated copies of the entire application, preferably
separated into folders or large envelopes. No Styrofoam packing peanuts!
